PRESCHOOL AND KINDERGARTEN
Preschool applicants must be three (3) years of age by September 1st and fully potty-trained by the start of school. Age-appropriate developmental observations/evaluations are conducted in a playgroup setting.

**We organize the January playgroup observations; St. Thomas will notify families of their observation time—all of which are assigned with care and backed by developmental research and best practices.
GRADES 1–5
Grade-level applicants are academically evaluated during their full-day St. Thomas Shadow Day Visit.**
Additional requirements include:
- School tour
- Completion of application
- Admission screening
- Copy of academic records for the current and previous year
- Teacher recommendation
- Shadow Day Visit (full school day that includes academic assessments and interview)
- Parent Meeting*
- Family photo
- Student photo
- Copy of student’s birth certificate
* All parents are required to attend the parent meeting.
**Shadow Day Visits occur during regular school days, between October and early February; dates are assigned by St. Thomas using a variety of internal factors that best sets Visitors up for success.

Immunization Requirements
St. Thomas mandates immunizations as required by the Florida Department of Health. Prior to attendance in our School, each student must have on file with St. Thomas a certificate of immunization for the prevention of communicable diseases, for which this department requires immunization. A completed Florida Certificate of Immunization (form DH680) is required to document the administration of prescribed immunization doses. An exemption from immunization requirements is permissible only with a physician's certification as to the need for either a temporary or permanent medical exemption. An exemption from these requirements is not permissible for religious, philosophical, personal, or other reasons.
